You can place the entity in the internal subset -- that part of the DTD
which is in the XML document itself. For example:

   <!DOCTYPE MyDocType SYSTEM 'mydtd.dtd' [
      <!ENTITY FirstName 'Kevin'>
   ]>
   <MyDocType>...</MyDocType>

Of course, the obvious disadvantage of this is that it needs to be
repeated in every XML document, but it's better than nothing.
