How about
   select="replace(., '&#12470;([^&#12540;])', '&#12470;&#12540;$1')"
?

And if that fails to catch &#12470; when it occurs at the end of a text
node, wrap the result in
    replace(., '&#12470;$', '&#12470;&#12540;')
