if you were using xslt2 you could just pass in teh table element node as
the third argument to key() which limits the scope to a single table.

in xslt1 the keys always have document-scope so the way to think of it
is that you don't want to just key off the id, you want tto key off a
combination of id and table so..

<xsl:key name="fnref" match="table[not(@print)]/tr/td/fn"
use="concat(generate-id(ancestor::table[1]),'/',@idref)"/>

...

...

key('fnref',concat(generate-id(ancestor::table[1]),'/',@idref))
