 Stopping any processing instruction output
I am doing some transformation in PHP5, but only pieces.  The end
result is containing the xml version processing instruction, which
would prefer it did not.  Here is the beginning of the XML:

<?xml version="1.0" encoding="UTF-8"?>
<EventData objectID="ca3d3b53-f880-4000-ac2b-616982215a02"
defaultOID="92a368c2-d96a-47a9-913a-a688fcab29df"
xmlns="urn://www.miltonstreet.com/events.xsd"
xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance"/>

Here is the beginning of the XSLT:

<?xml version="1.0" encoding="UTF-8" ?>
<xsl:stylesheet version="1.0"
xmlns="http://www.miltonstreet.com/events.xsd"
xmlns:xsl="http://www.w3.org/1999/XSL/Transform"
xmlns:p="urn://www.miltonstreet.com/events.xsd"
exclude-result-prefixes="p">

 <xsl:output method="xml" version="1.0" encoding="UTF-8" indent="yes"
standalone="yes"/>

And here is the output:

<?xml version="1.0" encoding="UTF-8" standalone="yes"?>
<div xmlns="http://www.miltonstreet.com/events.xsd" id="titlebar">Home</div>

How do I get ride of the processing instruction on the output?  Also,
is there any way to get ride of the xmlns attribute on the div?
