Something like this?

<xsl:template match="td[(position() mod 4) = 0]">
<entry      tag="{.}"
       attribute="{following-sibling::td[1]}"
             type="{following-sibling::td[2]}"
   description="{following-sibling::td[3]}" />
</xsl:template>
