You want:

<a>
  <xsl:attribute name="href">
    <xsl:text>lookup.php?word=</xsl:text>
    <xsl:choose>
       <xsl:when test="@lemma">
          <xsl:value-of select="@lemma"/>
       </xsl:when>
       <xsl:otherwise>
          <xsl:value-of select="."/>
       </xsl:otherwise>
    </xsl:choose>
    <xsl:text> target=iframewindow</xsl:text>
  </xsl:attribute>
  <xsl:value-of select="."/>
</a>

Or if you prefer:

<a href="lookup.php?word={(@lemma|.)[1]} target=iframewindow">
    <xsl:value-of select="."/>
</a>

(but don't use that unless you understand why it works...)
