Hi,

well perhaps the following would help

    <xsl:for-each select="item[@name=upper-case(@name)]">
        <xsl:sort select="@name"/>
        ...
    </xsl:for-each>

    and then the next for-each loop for all the lower-case names

    <xsl:for-each select="item[@name=lower-case(@name)]">
        <xsl:sort select="@name"/>
        ...
    </xsl:for-each>

Although this would mean, that no mixed upper and lower case characters in
the item attribute name
would be allowed.

Perhaps this could be resolved easier, but this is the first thing that came
to my mind..
