David Carlisle has pointed out to me that the PI target names "xml",
"XML" (etc) are not merely reserved, they also violate the grammar.
So <?xml as a PI (i.e. not at the start of the document where it's
an XML declaration) is a well-formedness error, as well as being
reserved.
