I'm not sure if your key question is "form" or "XSL", but if it's "form"
then you might be interested in XForms, which is probably about to become a
candidate recommendation (implementation phase).

Can anyone please point me to some samples of an
XSL web page with a form accepting incoming XML 
data, which then sends (POST) back XML data from the
XSL form?
