Rick,

Rather than look for a "form that accepts incoming data", you may want to look into a messaging protocol such as SOAP (Simple Object Access Protocol).  SOAP is an XML-based messaging protocol that runs on top of HTTP, SMTP, and other transport protocols (depending on the version) and carries a "payload" containing an XML-encoded message.  SOAP can be used to transport the incoming XML data (which ultimately is used in the form), and the XML data that was updated by user interaction with the form could be HTTP POSTed to where it needs to go.
