When writing XML output, the entities will not be named.
If you need them to have specific names, either you must
write your own serializer, or if you are using XSLT 2.0,
you can use xsl:character-map.

You must also make sure that the XML you generate has a
DTD associated with it which defines those entities if
you expect to be able to use the XML. If you are writing
just HTML, the DTD won't be needed.

<hov:output-character character="—" string="—"/>
<hov:output-character character="“" string="“"/>
<hov:output-character character="”" string="”"/>

John already answered that part.
You are using hex codes, forget the 'x' after the '#'.
Try "&#x____;" instead of just "&#____;"
