How can I have it simply copy each source
element after Record_id into the Target document without changing the
layout? I don't want to map each and every field because the fields
change with each new file. Some fields may exist some times and
sometimes they wont. What I currently have in the Stylesheet are the
only changes I need from my source file to my target file.
Thanks!

you need to do this through code:
<xsl:copy-of select="Record_id/following-sibling::*"/>

I still need some help with this. If you need to only make a couple of changes at the beginning of the file, actually, in the header record only, do you need to continue to map all the rest of the fields? I am looking for a way to have the Stylesheet only modify the header fields which need changing and then copy each record after that one for one to the end of the file creating the new formated xml file. How do I do this? Anyone??????

I forgot to create the attribute RECORD_ID, it should work now.

<?xml version="1.0"?>
<xsl:stylesheet version="1.0" xmlns:xsl="http://www.w3.org/1999/XSL/Transform">
<xsl:output method="xml" indent="yes"/>
<xsl:template match="/">
<HC_DATA>
<xsl:for-each select="HC_DATA/HEADER">
<HEADER>
<xsl:copy-of select="TRANS_CODE | RPT_YEAR | RPT_QTR | DATA_TYPE | DATA_TYPE | SUBMISSION_TYPE | SUBMISSION_TYPE | PROC_DATE | AHCA_NUM | ORG_NAME"/>
<CONTACT_PERSON>
<xsl:copy-of select="NAME | PHONE | STREET | CITY | STATE | ZIP | EMAIL"/>
</CONTACT_PERSON>
</HEADER>
</xsl:for-each>
<RECORDS>
<xsl:for-each select="HC_DATA/RECORDS">
<RECORD id="{RECORD_ID}">
<xsl:copy-of select="RECORD_ID/following-sibling::*"/>
</RECORD>
</xsl:for-each>
</RECORDS>

Thank you very much for your response. That appears to work fine.
