  Norm Walsh has his own way he used for example in the docBook stylesheets,
have a look at them.

Further to that, DocBook has specific supprt for documenting
computer software so it seems appropriate to use it for
XSL stylesheets (which are, after all, simply another form
of software).

This style of documentation is an essential part of
the engineering standards for the XSLT Standard Library
(http://xsltsl.sourceforge.net/).  There you can find quite
a few examples, as well as stylesheets to extract the
documentation from stylesheets and produce HTML,
all using XSLT instead of Make/Ant.
