I wrote something a little over a year ago to visualize DTD structure
using the JavaScript d3 toolkit, but it doesn't do editing. I suppose
in principle it could, but right now comments would be lost.

Actually I think it doesn't cope correctly with external entities in
all caes (if they override parameter entity definitions) but it could
probably be fixed. The DTD parser was a quick hack rather than a
product. But again, it could be fixed to use a proper XML parser fairly
easily. I was trying to explore different visualizations rather than
making a single robust one.
