Either use base64 encoded data inline in the XML (and stay away from DOM parsers), or use RFC 2387 multipart/related MIME encapsulation and use a URI in the XML that points to a CID: part of the message. See "XML-binary Optimized Packaging"  and "XForms Serialization as multipart/related"  as examples.
