Clearly, this is all true. That's why in real world applications, people forsake XML, HTML, and the common SGML conventions, relying solely on out-of-line markup. Seriously, out-of-line markup is useful, but is overkill for many applications. Ted seems to assume that only the requirements of his applications matter.
