It may or may not be the simplest solution depending on your exact 
requirements, but VoiceXML (http://www.voicexml.org) can do what you want.  
When you write your application, you write it as you would a Web application, 
using server-side scripting to access the database (usually JSP, PHP, ASP, 
Perl, or some other language).  VoiceXML uses forms and Javascript much like 
HTML does, and you can submit the keyed-in information using standard HTTP 
GET or POST requests.

VoiceXML has a lot of concessions for voice input, but it can also easily 
(perhaps more easily) accept DTMF (dial tone) input, which is I think what 
you are implying by "key in".

Hopefully the fact that VoiceXML is an XML language will keep it on-topic to 
this list, but there are far better places to find out more information.  
Look at voicexml.org to start.

I'm not sure exactly what you mean by this.  Generally a VoiceXML server (a 
"Voice Browser") answers telephone calls at a single number.

I'm afraid you will have to be much more specific as to your requirements 
before anyone will post code for you.  In the meantime, search around the web 
for some examples.  It is not terribly difficult, but no one will do your job 
for you.
