It is awfully easy with MS Access to build a database that works
fine locally, and won't scale.   The GUI tool is a boon though.  We
get hundreds of requests for ad hoc querying for our systems.  We
tell them that a third party report generator (Access, Crystal Reports, etc)
is the way to go.  ODBC is a pretty nice idea all things being equal.
While not DBAs, many of our customers really shouldn't
need to understand everything they are doing; they should understand
what they need to do.
 
But if the schema document [expletive deleted], even that falls apart.

Seems analogous to the argument that DBAs who use GUI tools don't really understand what they're actually doing.

Seems to me that RAD and drag and drop development leads you down
this path, you are developing so 'quickly' that you don't step
back and look at the architectural issues until it's too late.
