I think http://www.w3.org/2001/Annotea/ and the corresponding support in Amaya is trying to provide a general purpose annotation framework.  I haven't had a chance to look at it.  Anyone want to share
their experiences.

Also, Tim Bray's Annotated XML Recommendation (http://www.xml.com/pub/a/axml/axmlintro.html) was a pretty good example of what could be done.
