Greetings,

Currently, no complete XML exists for patient records, but some efforts are
underway. One is under ASTM,  E31, subcommittee 25, which is developing DTDs
for health care documents. In October of 1999 we held a joint meeting with the
Massachusetts Medical Society and developed draft DTDs for prescriptions,
discharge summaries, encounter notes, operative reports, diagnostic imaging
reports and the HCFA 1500 form for Medicare reimbursements. We are actively
seeking submissions to further the DTD development. www.astm.org.

A second effort is underway in HL7, Health Level 7 which is creating a patient
record architecture for exchange of clinical documents. www.hl7.org

The  two efforts are working together and I co-chair both activities.
