Docs for namespace and other processing instructions?

I have seen several references to the processing instruction "<?xml:namespace". I have also read the "Namespaces in XML" spec, which discusses the "xmlns:" attribute. The relationship between the two is not clear to me. My confusion may be connected to the fact that I have been unable to find any formal documentation of "<?xml:namspace" or other processing instructions. Is there any such formal documentation?
