Surrogate pairs are not allowed in parsed entities.  The production
for Char excludes the surrogate blocks:

[2] Char::= #x9 | #xA | #xD | [#x20-#xD7FF] | [#xE000-#xFFFD]
            | [#x10000-#x10FFFF]

You can include non-BMP/non-UCS-2 characters by making numeric
references to their Unicode Scalar Value (or by using UCS-4).
