XSchema: Naming vote count

The votes are in.  We are using:

Mixed Caps
No underscores
XML 1.0 specification names

The vote tallies were:

Capitalization:
---------------
ALL CAPS:      2
Mixed Caps:   17
no caps:       5

Underscores:
------------
Use:           6
Do not use:   18

Element and Attribute Names:
----------------------------
XML 1.0 spec: 13
Simple:       11
