DTD:

  <!ATTLIST foo

Document instance:

  <foo>...</foo>  
    The attribute "bar" has the value "hack", and is not specified
    (i.e., it is a defaulted value).

  <foo bar="hack">...</foo>
    The attribute "bar" has the value "hack", and is specified.

  <foo bar="hello">...</foo>
    The attribute "bar" has the value "hello", and is specified.
