All true enough, but if Peter is running under Cocoon (as I think he said), then Cocoon's serializer is the culprit and xsl:output is probably not being respected.

I think he needs to look at the serializer he is using in Cocoon. Assuming he wants XHTML, it's possible to construct one based on the XML serializer (if none is available out of the box: I can't recall).
