Rather than have to do all this ugly xsl stuff, why not expand use of
the <?dbhtml ... ?> processing instruction (PI) to allow such?  In
fact, I would like to see both db PIs expanded to allow arbitrary
attributes and values to be placed in generated html and fo output.
It would be up to the user to ensure the output was valid html or fo.
