I happen to be in a face-to-face meeting af the XSL FO subgroup, so
naturally we discussed your question, but I'm afraid that there is no hard
and fast answer for that.  Getting a spec to Recommendation depends on
resources -- and seemingly every XML-related WG at W3C, including ours, is
always on the lookout for more members -- and on there being
implementations of the spec.  Another fact of life for W3C specs is that
many people don't look at them or comment on them until they are at the
Candidate Recommendation stage -- the point at which the WGs are looking
for implementations -- and feedback at that late stage, while good, can
delay things more than if the feedback had been received earlier in the
process.

For XSL FO 2.0, itself, we are still working on new FOs in accordance with
our requirements, and I expect a new WD real soon now, then there will be
the point where we draw the line in the sand and say that's as much as we
can add to make a XSL FO 2.0.  On the implementation side, we don't yet
have implementations for most features, though an implementation could add
the features using an extension namespace if they don't want to yet use
the feature as 'official' FOs and properties.  On the feedback side,
including feedback from implementers, we'd like feedback as early is we
can get it so the spec is properly cooked when it does get to Candidate
Recommendation.
