Well I had read that a few times (and do a few tests), but got there
in the end... that's a real gotcha.

//doc//pb[1]

will return the first <pb> child of any descendant of <doc>, so you
could get many <pb>s for a single <doc>

The next time someone asks for questions for a test, that's a nice
(but unfair) one to suggest.
