as stated earlier for any XML file (not just XSL) if you use an entity
reference other than teh five built in ones you need to define it.

If your source file contained undefined reference and doesn't habe a
<!DOCTYPE that references a dtd then it is not well formed, and no XML
application will be able to process it.
