  For the identation, I would also copy the first child node if it is a
text node, before the added element.  That's not exact science, but
that should do the job if the input schema is properly idented:

    <xsl:copy>
       <xsl:copy-of select="node()[1][self::text()]"/>
       <xs:element ref=.../>
       <xsl:apply-templates/>
    </xsl:copy>
