  "note[count(para) eq 2]" or "note[para[2] and not(para[3])]".

  I think I remember Michael said once that Saxon rewrote the former
into the later (or maybe it was only "count(elem) = 2" into
"elem[2]"?), to avoid counting all the sequence items.  But I am not
sure.
