beware there never will be an XSLT 1.1, there was an early working draft
but it was killed off to focus (eventually) on XSLT2. So to write
standard conforming code you ought to move down to 1.0 or up to 2.0.

character references are replaced by the referenced character by the XML
parser before XSLt sees the input, you can't change this from XSLT as
it's happened before XSLt starts (and mandated by the XML spec)

XSLt can not tell how the character was input, but if you want to ensure
taht non ascii characters are output using references, use an output
encoding taht does not contain the characters, such as 
<xsl:output encoding="US-ASCII"/>

> disable-output-escaping="yes"/>
Don't use d-o-e ever is a good rule!
