preceding-sibling is one way of doing this:

  <xsl:for-each select="row[question_id!=preceding-sibling/question_id]">
    <xsl:variable name="q_id" select="question_id"/>
    Answers to question <xsl:value-of select="$q_id"/>
    <xsl:for-each select="../row[question_id=$q_id]">
      <xsl:value-of select="answer"/>
    </xsl:for-each>
  </xsl:for-each>
</xsl:template>
