  I'm having some trouble understanding how to count preceding-siblings that
  have different parents.


if you don't share a parent, you are not siblings.


Perhaps you want to use preceding::* rather than preceding-sibling
or perhaps you want to count your preceding siblings and your preceding
cousings (ie, the children of your parent's preceding-siblings)
count(preceding::*) + count(../preceding::*/*)

Most likely, you just want to use
<xsl:number level="any"/>

or since you are not actually using any of the hierarchy and are going
<xsl:apply-templates select="provider/claim/claim_line"/>
you can use
<xsl:value-of select="position()"/>
which is (unless MK says otherwise) likely to be considerably more
efficient than any of the above as it doesn't require anything to be
counted.
