XSLFO doesn't go into much of a detail when it comes to handling external graphics. Basically, it states "processor specific". Well, PDF can handle transparent PNG, and I thought Jimi (used by FOP) is able to transform PNG with alpha channel into the corresponding PDF data. You might have hit a PNG subformat which isn't supported by Jimi (it's old), or a bug. Try converting the PNG into a GIF first and see whether it works, or into another PNG subformat (preferably 16 bit color).
