The latter. Although a "purer" workaround, if you like, can be implemented in an XSLT system. (The required character is inserted by the XSLT serializer, not the transformation, but at least it's lights-out.)

Old distributions of James Clark's XT had a demonstration of how to do this. It may still be available. Googling for XT gets me:
