either by not selecting them in the first place by having a more
specific select=.... in your apply-templates,
or by throwing away everything that doesn't have a more specific
template with a template like

<xsl:template match="*"/>


In your second case, using a namespacem you still have
<xsl:template match="address">
which should of course be w:address
