I don't think this can be done in XSL-FO 1.0 because the side floats are rendered in the normal-flow-reference-areas as divided up by the column specifications of the body region in the page's span-reference-areas. Your picture will just overwrite the text in the second column. Would a float do that, if the picture was wider than one column, intruding halfway into column 2? No. What is the best way of 'intruding' an item into text, such that it repels the text like this? Side floats will do it within a given column, but will not have influence beyond the column.
