I've been using this form for some time, and follow it with
a mix of Chris's javascript to trawl for all xsl files in the project
directory, list them, then process them with a stylesheet to produce
a revision history.

Slightly different purpose, but similar goals.
My basic stylesheet now dumps:
<d:doc xmlns:d="rnib.org.uk/tbs#">
 <revhistory>
   <purpose><para></para></purpose>
   <revision>
    <revnumber></revnumber>
    <date></date>
    <authorinitials>DaveP</authorinitials>
    <revdescription>
     <para></para>
    </revdescription>
    <revremark></revremark>
   </revision>
  </revhistory>
  </d:doc>

from emacs for any new xsl file.
New revision elements are added as needed.

I've normally used xml comments (rather than <xsl:comment>)
before each template. I find context for named templates a must.
