worst entity possible
> How do I do this?
in general you don't.

the XPath data model used by XSLT has no type for entities.
entities, CDATA sections, white space inside tags, etc are just
regarded as syntactic features of the input file and are _not_ reported
to the XSLT engine.

It is possible (sometimes) to create an entity reference using
disable-output-encoding but it is never possible for an XSLt styleheet
to know what if any entity references were in the original file.
