Note that this only matches on an html element that is in no namespace.
If the source document is XHTML (as stated) then it will be in the XHTML
namespace, in which case the match will have to be against that.

ie
<xsl:template match="h:html">

where the h prefix is bound to http://www.w3.org/1999/xhtml
