HTML 4 should never have been in a namespace anyway, but it appears that you are creating elements in that namespace. If you are creating them in your stylesheet, just remove the xmlns="http://www.w3.org/TR/REC-html40 from there, and so produce html elements that are not in a namespace at all. If you are copying them from somewhere else then you'll need to write templates that convert from the namespace http://www.w3.org/TR/REC-html40 to the null namespace.
