You get the namespace wrong !

> <xsl:stylesheet version="1.0" xmlns:xsl="http://www.w3.org/TR/WD-xsl">

should be:

      xmlns:xsl="http://www.w3.org/1999/XSL/Transform" 

Additionaly, you need to add a similar template to handle the attributes
and should also take care if your documents and includinf namespaces
since name() gives you the full (qualified) name.
