how to get a count index in XSL

xsl index count
I want to keep track of the number of cards produced in WML, but it looks
it's impossible to do this with the xsl:variable or xsl:param since the
stored valued cann't be changed inside a template.  So how can I implement
a count so that I'll know I have produced 3 cards when I'm going to produce
the 4th one?

On the other hand, can a In-template binded variable be made Top-level so
other templates can use this variable?
