I think you are not appreciating the magnitude of your problem. You are marking up two document structures simulaneously, one with nested <div> elements, and one with totally unrelated <pb> elements (I guess this is TEI?). Now you want to combine the two. You really need to sit down and work out exactly what your _algorithm_ is going to be before you worry about implementation in XSL. You might get better answers on the TEI list.
