In a document without any DTD, a document with only an internal DTD subset which contains no parameter entity references, or a document with " standalone='yes' then an entity reference means that the document is not well formed and so it is a "fatal error" from which an xml parser is not allowed to recover, if there is an external dtd subset that has not been read then a reference is a validity error which means that a non validating parser id not obliged to report it.
