In a template I want to change the context node to the first child.

That's not a work-around, that's just the way to
do it using the "pull" approach.

This solution is ok, but if something like "xsl:with" or "xsl:context"
exists, I'd be glad to know.

Nope ... <xsl:for-each> is what you would use to
change context.  There shouldn't be multiple ways
of doing the same thing (though I agree it is
sometimes inevitable, but just because a word
doesn't sound right shouldn't warrant the duplication).
