More of a Xalan-J Users List question: http://xml.apache.org/mail.html#xalan-j-users; but generally speaking you'll find this error when you open an XML document in a text editor that doesn't properly understand the encoding, to then save it with that editors default encoding, resulting in an error similar to the one you are seeing.
