This sounds like you're having trouble with HTML? Or with PDF (do you
mean a form in a PDF file, or other output format)? How does your input
look like? How does your XSLT look like and how does your current output
look like?
In HTML, it will be something like"
<table>
<tr>
<td><input type="text" /></td>
<td><input type="text" /></td>
... 16 x....
</tr>
