I have a requirement to place a note "This page intentionally blank" on
any blank page. I've built a simple test case that forces an extra page
and can get that to work as expected, but when I try to apply it to my real
stylesheet and document, I get a blank page but no message.
Is the blank page model only triggered by specific circumstances and not other situations that cause a blank page?
